SCENARIOA male in his mid-sixties presented to his St Bryans Hospital emergency department (ED) with complaints of shortness of breath and chest pain. He had a history of chronic obstructive pulmonary disease (COPD) and cardiovascular disease (CVD). The patient was admitted for exacerbation of COPD symptoms and atypical chest pain. At various times during the admission process, the patient was noted to be a ‘falls risk’ but not fully recorded or assessed using a falls risk tool. He fell twice while in the acute ward in the am shift. He was then found later in the day on the floor near his bed, and was subsequently diagnosed with a fractured neck of femur. This resulted in the patient having to undergo a surgical procedure to treat the fractured neck of femur. The patient stated that he had used the call bell to request assistance getting to the bathroom but no- one responded. As a result, the patient attempted to get out of bed on his own and got caught in wires and cords that were attached to him, causing him to fall. On investigation of the incident, the team found that there were several safety issues that contributed to his fall. These included no bedside commode, no access to the bed alarm, no signage on the door or a wrist band. The number of falls on the ward has also increased over the recent months. As part of the St Bryans Hospital Best Practice committee you are required to research the current nursing literature about nursing management and prevention of falls. You will then present a report on your findings, and make recommendations for possible ‘nursing best practice standards or quality improvement strategies’ that would reduce the risk of falls for other patients in the facility.
